Collect responses in a channel where teachers cannot see who said what, summarise patterns rather than quoting individuals, and share the aggregate with the teacher before any departmental discussion.
When to Use This Template
Scheduled lesson observation
Share the observation focus and rubric with the teacher beforehand, record evidence against each dimension, and agree on a next observation date.
Annual staff evaluation cycle
Run self-assessment, observation and results sections through the same rubric, then collate scores and goals for the performance review meeting.
Agreeing goals for the next cycle
Close each evaluation by recording two or three specific, measurable goals for the next period and the support the teacher will receive, then revisit them at the following observation.
