How to Use This Template
Start with Observation Details: Teacher Name, Observer Name, Observation Date, Class Subject and Year Group, then set Next Observation Date before you leave the room. The teacher completes Teacher Self-Reflection Questions first — that ordering matters, because their answers should shape your Observer Evidence Notes.
Rate Rubric Dimension Ratings using your school's shared scale, and write Growth Goals (Collaboratively Set) together rather than announcing them. Both parties sign Signature Section, one copy goes to the teacher within five working days, and one stays with the line manager for the annual review.
Date-stamp each section as it is completed so the record shows reflection preceded judgement. If a rating is later disputed, Observer Evidence Notes written the same day carry far more weight than recollection weeks afterwards — file the form with the teacher's self-reflection attached rather than as two separate documents.
When to Use This Template
Scheduled lesson observation
Share the observation focus and rubric with the teacher beforehand, record evidence against each dimension, and agree on a next observation date.
Annual staff evaluation cycle
Run self-assessment, observation and results sections through the same rubric, then collate scores and goals for the performance review meeting.
Agreeing goals for the next cycle
Close each evaluation by recording two or three specific, measurable goals for the next period and the support the teacher will receive, then revisit them at the following observation.
