How to Use This Template
Assemble after the last observation of the year: fill Academic Year, Department and Line Manager, then set Overall Performance Rating consistent with your moderation outcomes. Summarise Observation Summary across the year's visits rather than the most recent one alone.
Use Student Outcomes for attainment and progress data, Professional Development for courses completed, and Contribution to School Life for clubs, trips and pastoral roles. Set Goals for Next Year with the teacher's input, and bring the completed summary to the review meeting as the basis for pay or responsibility discussions where applicable.
When to Use This Template
Scheduled lesson observation
Share the observation focus and rubric with the teacher beforehand, record evidence against each dimension, and agree on a next observation date.
Annual staff evaluation cycle
Run self-assessment, observation and results sections through the same rubric, then collate scores and goals for the performance review meeting.
Agreeing goals for the next cycle
Close each evaluation by recording two or three specific, measurable goals for the next period and the support the teacher will receive, then revisit them at the following observation.
