How to Use This Template
Enter Induction Start Date and Induction End Date, then name Mentor Name and Induction Coordinator so the support network is explicit on day one. Complete Assessment Schedule with the required assessment points across the induction period.
Set Induction Targets as achievable, observable goals and list Support Provisions — timetabled mentoring, reduced load, observation cycles. Both parties sign the Agreement section; review targets formally at each assessment point and keep evidence against them, because induction completion depends on documented progress, not goodwill.
When to Use This Template
