About This Template

The department's own review against standards: strengths, weaknesses, evidence and priorities for the coming year.

Everything runs in your browser — your school details are sent only when you choose Send. Export the finished document as an A4 or Letter PDF.

How to Use This Template

Fill Department Name, Head of Department and Evaluation Period, then write Student Outcomes with data and Quality of Teaching with evidence from observations and work scrutiny — claims need receipts.

Complete Curriculum and Resources honestly (gaps included), and set Improvement Priorities to two or three with measurable goals under each. Name Department Manager and sign off at Review Date. Use the form as the backbone of your department development plan rather than a separate document written for the visit.

When to Use This Template

Scheduled lesson observation

Share the observation focus and rubric with the teacher beforehand, record evidence against each dimension, and agree on a next observation date.

Annual staff evaluation cycle

Run self-assessment, observation and results sections through the same rubric, then collate scores and goals for the performance review meeting.

Agreeing goals for the next cycle

Close each evaluation by recording two or three specific, measurable goals for the next period and the support the teacher will receive, then revisit them at the following observation.

Frequently Asked Questions

How do I customise this template for my school?
Fill in each field with your school's specific information. The template has pre-defined sections that you can complete in any order. Your changes appear in the live preview on the right side of the screen.
Can I use this template for multiple students or classes?
Yes. Fill in the template once for each student or class, then download the PDF each time. You can keep the browser tab open and simply change the fields for different recipients.
Is my school data stored on your servers?
Template processing happens entirely in your browser — the filled content is never sent to our servers. When you download the PDF or send the template, your school details (name, country, and email or WhatsApp) are stored in our recovery store as described in our Privacy Policy. They are never shared or used for advertising.
How many criteria should a rubric include?
Four to six dimensions keeps the evaluation manageable — for example planning, delivery, assessment, differentiation and professional conduct. Too many criteria dilute feedback; too few miss important parts of practice. Rate each dimension on the same scale every time.
